The workshop fees cover:
o Admission to all Workshop sessions
o Workshop material; a book of abstract, a CD-ROM of presentation
files
o the welcome reception and the workshop banquet
o morning and afternoon coffee breaks
All payment must be made in Japanese Yen.

Secretariat via Fax or Postal mail. (PDF Registration

Confirmation for early bird and late advance registration:
Registrants who have made registration and payment will receive
a letter of confirmation and receipt after March 2, 2004.
This letter must be presented at the Registration Desk upon arrival.
At the registration desk, participants will
receive their Workshop documentation, abstract book.
On-site Registration Desk will be open: April
25 (Sun.), 2004 (5 pm to 7 pm) and during the normal Workshop
hours.
Cancellation and Refund Policy:
Cancellation must be made in a written form to
FIHFP2004 Registration Secretariat by Fax:+81-6-6456-4105
or E-mail:fihfp@jtbcom.co.jp.
If received before March 31, 2004, the
fees will be refunded after deduction of 5,000 Yen
to cover registration expenses. If received after
March 31, no refund will be made.
VISA: Should a visa be needed, participants should
contact FIHFP2004 secretariat
via e-mail by
January 30, 2004. Please note that the Secretariat cannot
prepare visa documents without confirmation of advance registration
for the Workshop.
Note that no financial
support for travel expresses and registration will be available
from the Workshop. |
